Allow specification of a unique header or footer text string to be defined for each page.

Some customers need to place unique text strings on each page of a document. This is common with Bates numbering. For example, page one might need a header with Jones0001, then on page two needs Jones0002. Using the existing page number option would help with this. However, some situations require a unique string, or numbering that does not start with one. Allowing specification of a header text string for page 1, a different header string for page two, etc. would provide great flexibility to our customers. Another example would be to change the header in major section breaks in a large document (if the customer knew where those breaks occur.)
